Patient Bill of Rights Revised to Support Staff Facing Discriminatory Behavior

Tim Lahey and Steven GrantFor the past several months, an 18-member task force headed by Timothy Lahey, M.D., MMSc (at far left), professor in the Division of Infectious Disease, and Steven Grant, M.D. (at left), associate professor in the Division of Hospital Medicine, has been working on support for staff managing unsafe patient behavior. From violent outbursts to rapidly escalating situations, the group has been helping make sure clinicians can be compassionate and safe at the same time.

This has led to a new policy and a pilot project of a Behavior Response Team on Baird 3 that in time is hoped to expand to the whole institution.

Along the way, team members realized that the Patient Bill of Rights and Responsibilities needed revision. That led to an effort led by Stephen Graves of Equity, Diversity and Inclusion as well as Ira Bernstein, M.D., chair of obstetrics and gynecology and to which Drs. Lahey and Grant and others also contributed. Read more.

"I Couldn't be Prouder:" The COVID-19 Response

Gilman Allen, Cindy NoyesAs the state of Vermont began to shut down in mid-March in response to the COVID-19 pandemic, members of the Department of Medicine mobilized for a potential influx of patients throughout the UVM Health Network.

Mark Pasanen, M.D., former chief of the Division of Hospital Medicine, says the effort to set up a system of care has relied on solid teamwork, trust, communication, and a good deal of flexibility. Although the pandemic is far from over, as many states see a resurgence of cases and community spread, Vermont has managed to keep case numbers low thanks in large part to the leadership and expertise of department members.

“Almost everyone has had to do something new on the fly,” he says. Pasanen served as chief of the Division of Hospital Medicine until July of 2020, when Caroline Lyon, M.D., associate professor, took over the position.

Gilman Allen, M.D. (above, left), professor in the Division of Pulmonary Disease and Critical Care Medicine, has been heading up the biologic branch of the incident command structure the state set up in the early days of Governor Phil Scott’s Stay Home, Stay Safe Order. Cindy Noyes, M.D. (above, right), associate professor in the Division of Infectious Disease and Amy Teleron, M.D., former assistant professor in the Division of Hospital Medicine, also both played key roles in this organization. Carney Co-Authors Commentary for Academic Medicine

Jan_CarneyJan Carney, M.D., M.P.H., professor in the Division of Pulmonary Disease and Critical Care Medicine and associate dean for public health and health policy, co-authored an invited commentary for Academic Medicine, the journal of the Association of American Medical Colleges, titled "Public Health is Essential: COVID-19's Learnable Moment for Medical Education." With Rika Maeshiro, M.D., she argues that this is a moment to "decisively incorporate public health, including public health systems, through the continuum of medical education."

"Although medical educators have made progress in integrating public health content into medical curricula, 'public health' is not a phrase that is consistently used in curricular standards, and public health colleagues are not identified as unique and essential partners to improve and protect health," the piece reads. Research and Grants

COVID-19 Research

Jason Botten, Ph.D.Jason Botten, Ph.D., associate professor in the Division of Immunobiology, and his research team are investigating ways to circumvent a major bottleneck to diagnostic detection of SARS-CoV-2 RNA by RT-qPCR, the mainstay test for active infection. The rate limiting step of this assay is the RNA extraction step. RNA extraction reagents are in limited supply, with many regions of the world unable to test due to shortages of these critical kits. Botten and team found that SARS-CoV-2 RNA detection can be done without an RNA extraction step. Thus, the “direct RT-qPCR” approach they discovered is rapid, cost effective, and available throughout the world by virtue of its non-proprietary platform and reagents. They are currently conducting a multi-country RING Trial of this technology in an effort to increase global COVID-19 testing capacity.

Jason Botten, Ph.D., associate professor in the Division of Immunobiology, and Renee Stapleton, M.D., Ph.D., professor in the Division of Pulmonary Disease and Critical Care Medicine, have partnered with the UVM Vaccine Testing Center, and Atreca and IgM Biosciences to map the repertoire of antibodies generated during acute human SARS-CoV-2 infection with the goal of identifying therapeutic monoclonal antibodies. Botten and Emily Bruce, Ph.D., faculty scientist in the Division of Immunobiology, have partnered with Dr. Keith Jerome’s lab at UW in Seattle (he leads all diagnostic efforts for the Pacific Northwest) and Dr. Syril Pettit’s Health and Environmental Sciences Institute (HESI) (her agency brings medical technology, treatments, and assays to those in need) to improve global COVID-19 testing capacity. Botten’s team has also begun working with Dr. Dev Majumdar (UVM Surgery) to study SARS-CoV-2 interactions with host cellular machinery and Chris Huston, M.D., professor in the Division of Infectious Disease, to screen antiviral compound libraries against SARS-CoV-2 (the latter funded, in part, by NASA).

As a response to the COVID-19 pandemic, Mary Cushman, M.D., M.Sc., professor in the Division of Hematology/Oncology, and others planned and launched the rapid COVID COAG trial over about a two-month period. The trial is testing effectiveness of full anticoagulation with heparin or low molecular weight heparin in medical ward patients with evidence of coagulopathy. Over 40 hospitals in five countries are involved in the activation process.
