Current and Future State of Performance Change
Most practices at this time have access to behavioral health treatment for mental illness through referral to on- or off-site counselors, social workers or health coaches. Uptake of these services is limited unless they are fully integrated into the practice's culture and recognized through value-based payment.
In the future, all advanced primary care should include full, easy on-site and telehealth access to mental health conditions, behavior change counseling for chronic disease management, and substance use disorder care including screening, medication-assisted treatment and counseling.
The primary care team of the future will fully utilize the skills of psychologists, social workers, nurses, and physicians who have a high degree of interprofessional skill that maximizes efficient use of their time, provides the best possible measurable outcomes for patients, high provider satisfaction, and lower costs of care.
Objectives
- Gather the information needed to lead their practices toward some degree of integrated behavioral health.
- Appreciate the role of a team-based approach to integrated behavioral health. Advocate for patients to receive the care they need.
- (Psychology/mental health clinicians) Learn about the varying roles for BH clinicians within primary care practices. Identify how to best work collaboratively with referring PC clinicians.
Audience
Target audience includes: Physicians, Nurses, Nurse Practitioners, Physician Assistants, Mental Health Counselors, Psychologists, Social Workers, Alcohol and Drug Abuse Counselors, and other health care providers with an interest in integrated practice.
