Course Description

This year, Transforming Primary Care will provide strategies for prioritizing and measuring behavioral integration into medicine. Leadership in the National Committee for Quality Assurance (NCQA), OneCare, and Community Physicians and Behavioral Health practitioners involved in integration activities will discuss specific strategies to help your practice improve medical care, moving Vermont healthcare forward clinically and financially.

Participants will review practice assessment, recognize facilitators and barriers to integration, consider the need for a Vermont interprofessional training model for those in practice and in Vermont academic institutions. An optional workshop day will facilitate practice-specific integration improvement strategies using the Behavioral Health Integration Toolkit. Join us!

Objectives

  • Identify 3 emerging areas of importance for behavioral health elements as part of regulatory and financial elements of healthcare reform.
  • List the elements of an integrated model of medication assisted treatment of narcotics dependence.
  • Discuss the range of personnel needed for training in team-based care.

Abstracts Requested

We invite participants to submit an abstract to present a poster at the conference. We are specifically asking for the abstracts to describe research quality improvement projects or innovations related to Integration. The blinded abstracts will be judged and ranked. Six abstracts will be chosen and registration fees will be waived for the winners. Poster abstracts will be accepted until September 16. The poster abstract form (see link above) may be uploaded upon conference registration online. Those submitting abstracts should complete the conference registration form.

Please plan to bring your laptop or electronic device to view presentations as well as completing the electronic conference evaluations via our online UVM CME system. Printed syllabus, paper copies of evaluations and paper certificates will no longer be available. You will receive a notification (via the email you used to register for this conference) about a week from the conference start date that presentations will be available for downloading.

Accreditation

The Robert Larner, M.D. College of Medicine is accredited by the American Nurses Credential Center (ANCC), the Accreditation Council for Pharmacy Education (ACPE), and the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing education for the healthcare team.

The University of Vermont designates this live activity for a maximum of 14 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

The National Association of Social Workers, Vermont Chapter, has approved this program for 14 continuing education credits for social workers in Vermont. The vendor number for this training is 02-042502-10202016.

The Vermont Board of Mental Health Counselors has approved this program for up to 14 CEUs.

The Vermont Board of Pharmacy has approved this program for fourteen (14) hours of live (didactic) pharmacy continuing education credit. The approval number issued is CPE012(L)-0824.

The Vermont Board of Psychological Examiners has approved this program for up to 14 CEUs.

Credit is pending for Pharmacological Credit.

This conference includes .75 credits in Prescribing Controlled Substances AND 1.5 credits in Hospice, Palliative Care, Pain Management.

This program has been reviewed and is acceptable for up to 14 Nursing Contact Hours.

PLEASE NOTE: If you do not participate in the optional workshop, the conference is only accredited for 7.5 credits.

Refunds

Refunds may be obtained, minus a $50 administrative fee, if notice of cancellation is received by October 10. Refunds are not available after that date.

The conference will be held at The Essex in Essex Junction, Vermont, located at 70 Essex Way. A small block of rooms has been reserved at The Essex for conference participants.
