COURSE DESCRIPTION
How has your practice integrated behavioral health services to-date? What are your ongoing needs? This year, the Transforming Primary Care conference will focus on strategies for implementation. National leaders in the field will give an overview of what integrated behavioral health means; will describe how full implementation has worked in a large urban medical center for a pediatric population; and will demonstrate how the AIMS Center helps practices move toward collaborative care.
Additional sessions will provide examples of how behavioral healthcare has been integrated within different practices in the Vermont region. Speaker panels will be an opportunity for participants to ask questions relevant to their own practices, and breakout sessions will give us the opportunity to go deeper into the process of integration. Recognizing that we learn best when our minds are refreshed, we will begin break sessions with wellness activities to restore focus and take care of our own health.

OBJECTIVES
- Describe different approaches to the integration of behavioral health services within primary care.
- Describe the importance of a team-based approach (including nursing and office staff) to fully integrated care.
- Identify strategies that would work in YOUR practice to improve care of chronic medical illness and mental health conditions.

CONFERENCE REGISTRATION
$275 for physicians, $185 for other healthcare professionals and $75 for students. The fee includes the electronic download of course syllabus, continental breakfast, lunch and coffee breaks each day. Please register by August 14. An additional $50 will be added after that date.
Refunds may be obtained, minus a $50 administrative fee, if notice of cancellation is received by August 14. Refunds are not available after that date.
CONFERENCE LOCATION
The conference will be held at The Essex in Essex Junction, Vermont, located at 70 Essex Way. The Essex, “Vermont’s Culinary Resort & Spa,” is Burlington area’s only AAA Four Diamond property. The resort, featuring on-site golf, ropes course, pond, and much more, is set on 18 acres of Vermont countryside. The 22,000 square foot, full service spa includes 10 treatment rooms, a 25-yard indoor pool, outdoor hot tub, fitness center, hair and nail salon, and locker rooms with steam rooms, saunas and relaxation areas.
